On page 1 of his classic text, Millsap states, “Measurement invariance is built on the notion that a measuring device should function the same way across varied conditions, so long as those varied conditions are irrelevant [emphasis added] to the attribute being measured.” By construction, measurement invariance techniques require not only detecting varied conditions but also ruling out that these conditions inform our understanding of measured domains. In fact, measurement invariance techniques possess great utility when theory and research inform their (...) application to specific, varied conditions that, if not detected, introduce measurement biases, and, thus, depress measurement validity. Yet, we see emerging bodies of work where scholars have “put the cart before the horse” when it comes to measurement invariance, and they apply these techniques to varied conditions that, in fact, may reflect domain-relevant information. These bodies of work highlight a larger problem in measurement that likely cuts across many areas of scholarship. In one such area, youth mental health, researchers commonly encounter a set of conditions that nullify the use of measurement invariance, namely discrepancies between survey reports completed by multiple informants, such as parents, teachers, and youth themselves. In this paper, we provide an overview of conceptual, methodological, and measurement factors that should prevent researchers from applying measurement invariance techniques to detect informant discrepancies. Along the way, we cite evidence from the last 15 years indicating that informant discrepancies reflect domain-relevant information. We also apply this evidence to recent uses of measurement invariance techniques in youth mental health. Based on prior evidence, we highlight the implications of applying these techniques to multi-informant data, when the informant discrepancies observed within these data might reflect domain-relevant information. We close by calling for a moratorium on applying measurement invariance techniques to detect informant discrepancies in youth mental health assessments. In doing so, we describe how the state of the science would need to fundamentally “flip” to justify applying these techniques to detect informant discrepancies in this area of work. (shrink)

RationaleSurrogates of critically ill patients often have inaccurate expectations about prognosis. Yet there is little research on how intensive care unit clinicians should discuss prognosis, and existing expert opinion-based recommendations give only general guidance that has not been validated with surrogate decision makers.ObjectiveTo determine the perspectives of key stakeholders regarding how prognostic information should be conveyed in critical illness.MethodsThis was a multicenter study at three academic medical centers in California, Pennsylvania, and Washington. One hundred eighteen key stakeholders completed in-depth semistructured (...) interviews. Participants included 47 surrogates of adult patients with acute respiratory distress syndrome; 45 clinicians working in study ICUs, including physicians, nurses, social workers, and spiritual care providers; and 26 experts in health communication, decision science, ethics, family-centered care, geriatrics, healthcare disparities, palliative care, psychology, psychiatry, and critical care.Measurements and main resultsThere was broad support among surrogates for existing expert recommendations, including truthful prognostic disclosure, emotional support, tailoring the disclosure strategy to each family's needs, and checking for understanding. In addition, stakeholders offered suggestions that add specificity to existing recommendations, including: In addition to conveying prognostic estimates, clinicians should help families "see the prognosis for themselves" by showing families radiographic images and explaining the clinical significance of physical manifestations of severe disease at the bedside. Many physicians did not support using numeric estimates to convey prognosis to families, whereas many surrogates, clinicians from other disciplines, and experts believed numbers could be helpful. Clinicians should conceptualize prognostic communication as an iterative process that begins with a preliminary mention of the possibility of death early in the ICU stay and becomes more detailed as the clinical situation develops. Although prognostic information should be initially disclosed by physicians, other members of the multidisciplinary team-nurses, social workers, and spiritual care providers-should be given explicit role responsibilities to reinforce physicians' prognostications and help families process a poor prognosis emotionally.ConclusionsFamily members, clinicians, and experts identified specific communication behaviors that clinicians should use to discuss prognosis in the critical care setting. These findings extend existing opinion-based recommendations and should guide interventions to improve communication about prognosis in ICUs. (shrink)

In 2018-2019, at the Keck School of Medicine of the University of Southern California, we developed and piloted a narrative-based health systems science intervention for patients living with HIV and medical students in which medical students co-wrote patients’ life narratives for inclusion in the electronic health record. The pilot study aimed to assess the acceptability of the “life narrative protocol” from multiple stakeholder positions and characterize participants’ experiences of the clinical and pedagogical implications of the LNP. Students were recruited from (...) KSOM. Patients and staff were recruited from the Maternal, Child, and Adolescent/Adult Center for Infectious Disease and Virology at Los Angeles County+USC Medical Center. Ten patients, seventeen students, and ten MCA staff participated in the pilot study. Qualitative methods were used to gather data from students’, patients’, and staff’s perspectives. Three themes emerged from the thematic analysis: patients’ life narratives conveyed their unique life experiences and voices; the protocol could result in wide-ranging effects on HIV care; the LNP enabled students to contribute value to patients’ healthcare. Across groups, participants considered the LNP an acceptable intervention. The LNP, its limitations, and implications for HIV care, narrative medicine, and health information technology are presented. (shrink)

The use of Covariance Analysis to show the pure effect of treatment free from the effect of collateral variables is also indicated. Nested ANOVA that allows division of ANOVA in several parts in order to answer complex questions is described and explained. Este artículo busca demostrar que, a partir de los lineamientos presentes en el capítulo II del reciente _Reprise du donné_ es posible proponer cuatro niveles de hermenéutica que nos permiten resolver algunos de los problemas de la fenomenología marioniana lúcidamente planteados por Christina Gschwandtner.

Nuestra aproximación al fenómeno saturado en J.-L. Marion se realizará mediante los ejemplos de mandato y anuncio que se dan en el ámbito de la revelación. Con ellos justamente se pretende llamar la atención sobre la necesidad de una fenomenología de la donación que abarque lo que desde nuestra comprensión humana se rechaza como intolerable, e incluso imposible, atendiendo sin embargo a fenómenos saturados que no se limitarían a un determinado ámbito, como algunas críticas parecen sugerir cuando se refieren al (...) “giro teológico” en la fenomenología (Janicaud, 1991). La donación exige ser reconocida, y que no nos opongamos a ella de entrada por nuestras inclinaciones metafísicas hacia la objetividad, el cálculo, el control, la predicción, etc. En este sentido nos referimos a un sacrificio de la metafísica, acreditando la oportunidad para que pudiera desmentirse lo que parece hacerla imprescindible (más que meramente necesaria), y a favor de nuevas posibilidades del pensar y del enriquecimiento de la experiencia. Para ello sería de suma relevancia proteger la confianza en la donación, incluso en los casos más extremos, que en nuestros ejemplos también dan crédito a la libertad. (shrink)

In 1561 J.Cornarius proposed his own version of some passages of Plato's Sophist In this version Theodorus presents the Eleatic Stranger as "a companion (hetairos) of Parmenideans and Zenonians" (216a). Since then, this cliché is accepted by all translations. However, when the possibility of justifying the existence of images and appearances is considered, the Stranger himself proposes 'testing' Parmenides' thesis. His remarks are rather those of an adversary than of a friend or companion of Parmenides. In fact, in spite of (...) Theodorus' presentation, the Stranger, albeit citizen of Elea, does not seem to share the theses of the 'Eleatics'. These anomalies invited us to question the character of 'companion' of the 'Parmenideans' credited to the Stranger. The questioning is possible if we exploit some valuable greek manuscripts of Plato's Sophist, neglected by J.Burnet, like Vindobonensis 21 (Y). This manuscript, among others, has the lecture heteros, 'different', instead of hetairon, 'companion'. This manuscript permits to maintain the formula tôn hetairôn, transmitted by all the manuscripts after the first hetairon, and removed in modern editions. The translation we propose is: the Eleatic Stranger is "different (heteros) of the companions (tôn hetairôn) of Parmenides and Zenon". (shrink)

Rome and the Enemy: Imperial Strategy in the Principate. Berkeley and Los Angeles: University of California Press, 1999. xxi + 259 pp. Cloth, $35.00. This well-written book is nothing if not bold. Its time frame is, broadly speaking, the first two and a half centuries A.D. Its basis is the view that has met increasingly (...) wide acceptance over the past quarter-century, that the Romans' conduct of foreign relations was altogether different from what we assume of modern states. The Roman mindset, motivation, intelligence, and strategy are all alien to us and thus require evaluation on their own terms. Consequently, in five chapters Mattern first examines the character and education of Rome's small decision-making elite. She next moves to their "image of the world," variously gained through campaigning, geographical knowledge, intelligence, and ethnology. A chapter on strategy follows, treating the size and deployment of the Roman army and the different means by which the empire was defined and defended. Income and expenditure are the focus of the fourth chapter, with particular reference to the expense of maintaining the army, the cost of warfare, and the economic gain to be made from conquests. Finally a chapter on "values" concludes the book--the unashamed glorification of conquest, the prestige accruing to the Romans in general and to the emperor in particular, and reinforcement of the sense that it was discipline, both moral and organizational, that made Rome supreme over other peoples. [End Page 451]The discussion of all these aspects displays insight and balanced judgment, and the accompanying bibliography is valuable (some notes are repetitious, however: cf. 117 n. 163 with 172 n. 36). The work of Ampelius (ed. Budé 1993) might have been cited (25) as another example of how weak even a schoolmaster's geographical knowledge could be. Although, to be sure, it is true that "bird's-eye view" was hardly an option for Roman representations of the world (26), we might still recall Artemidorus' (Oneirocritica 2.68) vision of dreamers flying along and looking down at the landscape below, while the presentation of Rome's Marble Plan is large-scale and ichnographic. The Tabula Banasitana (AE 1971.534) merits citing as a prime example of Marcus Aurelius' pondering with care the bestowal of benefits upon people on a remote fringe of the empire (180). In stating that we shall never know where the Teutoburg Forest was, let alone the site of Varus' defeat (167), Mattern seems to overlook the finds made at Kalkriese (see now W. Schlüter in J. D. Creighton and R. J. A. Wilson, eds., Roman Germany: Studies in Cultural Interaction [1999] 125-59); equally, she shows no awareness of a kernel of truth in the tradition that Hadrian abandoned Dacia, at least in part (104; cf. Barrington Atlas, Directory, 1376). In treating the Roman concern to save "face" (177), there is more to be made of L. Caesennius Paetus as scapegoat. The major step of annexing Armenia must surely reflect instructions he brought with him from Rome, rather than just a personal preference (cf. 92 n. 46), but all the blame for the rapid, catastrophic failure of the annexation attempt is then heaped upon him alone. Tacitus grasped Rome's vulnerability in this region. He has the Parthian cavalry commander Vasaces coolly dismiss Paetus' bluster about long-standing Roman control there: "imaginem retinendi largiendive penes nos, vim penes Parthos" (Ann. 15.14). Later, after Corbulo's diplomatic victory, Tacitus can detail the official tokens of respect insisted upon by Tiridates during his journey to Rome and conclude ironically: "scilicet externae superbiae sueto non inerat notitia nostri apud quos vis imperii valet, inania tramittuntur" (Ann. 15.31).These and other sundry observations aside, it is the book's principal thesis that invites comment, namely Mattern's sweeping claim: "I would argue that Rome's real strategy lay in the realm of psychology. Cosmopolitanism in many (...) periods (including the present post–Cold War era) has fostered political internationalism—schemes of world government, federal unions, and mechanisms for peaceful resolution of interstate conflicts. Although the Hellenistic age enjoyed little respite from war, some scholars have hailed Greek arbitration as a model for modern imitation.A brief survey of previous scholarship on arbitration will place Ager's work in perspective. After Husserl and Heidegger, phenomenology has attempted to push the reduction beyond the reference of objects to the performances effected by consciousness, or of beings to Being. First, a new level of the reduction comes forth in M. Henry's radical reduction of appearing to the appearing of appearing, and leads to the disclosure of a dimension in which no horizons are to be fulfilled because the superabundance of life holds sway. Secondly, according to H. Rombach, the phenomena decribed in the (...) first two levels are nothing but epiphenomena, and must be explained in terms of structures that emerge in a creative development. Reality brings forth something from itself, and we must take advantage of this contribution in order to obtain more than what is anticipated in our projects. Thirdly, J.-L. Marion advances a theory on saturated phenomena in order to account for phenomena that surpass the bounds outlined for them in the mere significational intentions because they show a surplus of intuition. The three trends lead respectively to an originary phenomenality, an outbreak of phenomena, and saturated phenomena. The article contends that they tend to come together because they link the third level of phenomenology to a reflection that turns on the duality between what appears and its appearing, and, in doing so, disclose a surplus with regard to three different aspects of the duality, i.e., appearing as such, the overall condition of what appears, or the appearing of some preeminent phenomena. Las tres tendencias conducen respectivamente a una fenomenalidad originaria, a un estallido del fenómeno y a una saturación del fenómeno. El artículo sostiene que tienden a converger porque vinculan el tercer nivel de la fenomenología con una reflexión en torno de la dualidad entre lo que aparece y su aparecer, y que, al hacerlo, ponen de manifiesto una excedencia respecto de tres aspectos distintos de la dualidad, esto es, el aparecer en cuanto tal, la condición general de lo que aparece o la aparición de algunos fenómenos preeminentes. Finalmente, se indican algunas anticipaciones de estas tendencias en la fenomenología trascendental de Husserl. (shrink)

In this paper, I defend H. L. A. Hart against two prevalent criticisms of his views on social rules and the obligations with which these rules are often associated. These criticisms, I argue, rely on misunderstandings ormischaracterizations of what Hart actually intended. These misunderstandings are plausibly explained by a failure on the part of his critics to appreciate fully two of the valuable lessons Hart sought to communicate in his inaugural lecture. First, words like ‘rule’ and ‘obligation’ should not be (...) removed from their various contexts of use and subjected to abstract philosophical definition and analysis. L. A. Hart de dos críticas recurrentes a su teoría de las normas sociales y a las obligaciones que regularmente acompañan a estas normas. Ambas críticas, sostiene el autor, descansan sobre malos entendidos o caracterizaciones equivocadas de lo que Hart en realidad se propuso. Estos malentendidos son explicados plausiblemente a través de la falta de capacidad de los críticos de apreciar plenamente dos de las valiosas lecciones que Hart deseaba expresar en su lectura inaugural. Este artículo procura establecer lo rasgos distintivos de los tres estadios de la reducción erótica formulada por J.-L. Marion en contraste con la reducción epistémica al cogito y la reducción ontológica al ser. En primer lugar, en la búsqueda de una seguridad que involucra al otro, la autocerteza del cogito es reemplazada por la pregunta: "¿se me ama de otra-parte?". En segundo lugar, como esta pregunta no puede ser respondida recurriendo al amor a sí mismo, se impone una formulación radicalizada: (...) "¿puedo amar, yo el primero?". Este nuevo estadio da lugar al análisis de fenómenos en que cada participante da al otro lo que no tiene de suyo y al mismo tiempo tiene a su disposición solo para ofrecerlo al otro, esto es, el significado, la carne, el juramento, el gozo, la fidelidad, y el adiós=a Dios. En tercer lugar, puesto que esta lógica del amor muestra la prioridad del otro, se presenta la formulación final de la reducción erótica: "¡Tú, tú has amado el primero!". Marion sostiene que, a través de esta precedencia, se revela el amor de Dios. Una apreciación final de las contribuciones de Marion esboza superposiciones y contrastes con otras posiciones fenomenológicas. This article sets out to establish the distinctive traits of the three stages of the erotic reduction advanced by J.-L. Marion in contrast to the epistemic reduction to the cogito and the ontological reduction to Being. First, in the search for an assurance involving the other, the self-certainty of the cogito is replaced by the question "¿Am I loved from elsewhere?". Second, as the question cannot be answered by the way of self-love, a radicalized formulation must be given: "¿Can I love as the first one?" This new stage brings forth the analysis of phenomena in which each partner gives the other what he does not have on his own and at the same time has at his disposal only to offer it to the other, i.e., meaning, flesh, pledge, joy, faithfulness, and good-by=God be with you. Third, as this logic of love discloses the priority of the other, a final formulation of the reduction is afforded: "¡You, you have loved as the first one!" Marion contends that, through this precedence, God's love is revealed. A final assessment of Marion's contributions outlines overlappings and contrasts with other phenomenological positions. El artículo comienza con una breve referencia a la introducción de la fenomenología de Merleau-Ponty en la Argentina. Se consideran primero las contribuciones de Luis M. Ravagnan y Carlos L. Ceriotto para pasar luego a un examen de las referencias efectuadas por Luis Juan Guerrero en su _Estética_. En relación con la estética de las manifestaciones artísticas se pone énfasis en la noción merleau-pontyana de lenguaje indirecto y en la significación de las voces del silencio. En lo que concierne a (...) la estética de las potencias artísticas se pone el acento en la experiencia no-objetiva de convivencia en la comprensión mutua, a fin de dar cuenta de la respuesta del artista al llamado de la tradición cultural y de la respuesta del espectador al llamado de la obra de arte. Finalmente, respecto del problema de las tareas artísticas, la atención se focaliza en el análisis que Guerrero, nuevamente siguiendo a Merleau-Ponty, ofrece sobre las fiestas cuyo rasgo distintivo es ser el gran juego de pueblos históricos y el lugar originario de reunión de todas sus tareas artísticas.
